Outside Activity Student Release Form

I understand by submitting this form I release KingCare of responsibility for my child these days and times. I understand it is my responsibility to speak to my child about proper procedures in the event there is no adult present at the start of said activity. It is also my responsibility to inform KingCare if my child’s activity has been cancelled or rescheduled. If my child will be returning to KingCare after the activity, I understand that KingCare staff will not be able to go to the location to retrieve my child. I will be responsible for instructing my child to walk directly from the activity to the Gym to check in with KingCare.
